Prominent Cheekbones? Why Surgery Isn’t Always Necessary

Prominent Cheekbones? Why Surgery Isn’t Always Necessary
By Dr. Yoorim Choi of Dr. Sonyouna Clinic

Whenever you look in the mirror or take a photo, do your cheekbones seem unusually prominent, making your face appear flatter and wider from the front? Many people quickly assume, “Is it because my facial bone structure is too large?”

However, a detailed facial analysis often reveals that prominent cheekbones are not caused by bone size alone. The position, volume, and shape of the fat pads over the cheekbone area can have an even greater impact on facial contour, depth, and overall balance.
Because cheekbone structure varies from person to person, cheekbone reduction surgery or bone shaving may not always be necessary. In some cases, targeted fat-dissolving injections can help reduce excess fullness around the cheekbones and create a slimmer, more balanced facial contour without surgery.
In this article, I will explain the different areas of the cheekbones that are commonly discussed during facial contouring consultations.

1. Front Cheekbones: The Key to Facial Dimension and a Youthful Appearance

Front Cheekbones: How to Determine Whether Cheekbone Reduction May Be Needed

The area commonly known as the apple of the cheeks, extending from beneath the eyes down toward the nasolabial folds, is referred to as the front cheekbone area or anterior cheek area. This region plays an important role in creating facial dimension and a bright, youthful appearance at the center of the face.
However, having fuller apple cheeks does not necessarily mean the face will look more attractive. The most important factor is not simply the amount of cheek volume, but where that volume is positioned.

  • Ideal Characteristics: The cheek area extending from beneath the eyes toward the sides of the nose should have a balanced amount of soft, natural fullness and gentle projection. This helps create greater facial dimension, soften the overall appearance, and give the face a naturally youthful look.
  • When to Be Cautious: If the cheek tissue in this area begins to sag and accumulate directly above the nasolabial folds, excessive volume can make the folds appear deeper and more pronounced. It may also draw the facial contour downward, making the lower face look longer and the overall appearance seem older than the person’s actual age.

2. Lateral Cheekbones: The Key Area That Defines Facial Width and the Outer Facial Contour

Lateral Cheekbones: How to Determine Whether Cheekbone Reduction May Be Needed

The lateral cheekbone area refers to the outer facial contour extending from the outer corner of the eye toward the area in front of the ear. When viewed from the front, this region plays a major role in determining overall facial width and defining the outer facial contour.

  • Ideal Characteristics: The contour extending from the lateral cheekbones down toward the jawline should form a soft, gently curved line with balanced facial width. This helps preserve natural facial dimension and a bright, youthful appearance.
However, I do not recommend reducing the volume of the lateral cheekbones until the area becomes completely flat, as this may visually elongate the face and disrupt overall facial balance.
  • When to Be Cautious: If there is excessive fat volume over the lateral cheekbones, the outer facial contour may appear uneven or wavy rather than smooth and well-defined. This excess fullness can also make the face look wider and flatter from the front, while visually emphasizing hollow cheeks. As a result, the overall facial appearance may seem sharper, harsher, or more severe.

3. The Key Is Achieving Balanced Proportions Across the Entire Face

In summary, an attractive face does not necessarily require the smallest possible cheekbones. Achieving a smooth, refined facial contour requires careful and personalized facial design, with overall facial balance as the primary consideration.
The key is to determine which areas would benefit from volume reduction and which areas should be preserved to maintain natural depth, dimension, and harmony across the entire face.

For individuals with an uneven or wavy facial contour, or excess cheek tissue that gathers above the nasolabial folds and makes the cheekbones appear overly prominent, targeted fat-dissolving injections may offer an effective and precise facial contouring option.
This non-surgical treatment is designed to reduce localized fat in specific areas without the risks, recovery time, and concerns associated with cheekbone reduction surgery or bone contouring. By selectively reducing unnecessary fat deposits while preserving essential facial volume, the overall facial line can appear smoother, softer, and noticeably slimmer.

If you want a beautifully balanced facial contour while preserving the unique features that make you look like yourself, I recommend consulting an experienced medical professional for a detailed facial structure assessment before choosing any treatment.
A personalized evaluation can help determine which areas should be refined, which volume should be preserved, and which facial contouring approach is most suitable for your individual features.
